Medications at School

Medication administration at school is highly regulated by Massachusetts law. The medication policy is outlined in your parent handbook.

If your child will require prescription medication at school, please follow these steps:

1. Obtain an order/emergency care plan from your child’s doctor. Often, the doctor’s office will send these items if you ask. Confidential school nurse fax numbers are listed here: www.somerville.k12.ma.us/nursing
2. Meet with the school nurse to make a plan for school and field trips
3. Bring the medication to the school nurse in a pharmacy-labeled container
4. Complete the Medication Administration form below

Required Forms For Prescription Medication Administration

If your child will require medication at school, please fill out this Medication Administration Plan Form. Fill out online or follow instructions to print the form and bring it in to your child's school nurse.

Certain medications may be self-administered with parent or guardian and school nurse permission. These medications are restricted to the following four: asthma rescue inhalers, epinephrine auto-injectors, enzymes for cystic fibrosis, insulin and glucose testing devices. Other medications can be considered on a case-by-case basis with the school nurse. The parent or guardian must fill out the Medication Administration Plan Form above, which includes a Self-Administration Plan, and they must meet with the school nurse before any medication can be self-administered. 

Required Form for Yearly Medical Update

All families must fill out the Yearly Medical Update Form annually. It includes prompts for medical changes as well as the over-the-counter medication permission form. Students may not carry over the counter medication such as Tylenol or Motrin, but the school nurse may administer it with parent / guardian permission.
