Under-5 COVID-19 Vaccine Event

CAAS Head Start and the Somerville Department of Public Health present this free Under-5 COVID-19 vaccine event. This event is for all children ages 6 months to 4 years 11 months. By appointment only. Please sign up using the following link: https://hipaa.jotform.com/222215765096155

The vaccine given will be Moderna.

After choosing an appointment time, please download and fill out the available consent forms and screening questions prior to your appointment. If you have health insurance, please bring your insurance cards to your appointment. Health insurance is not needed to receive the COVID-19 vaccine.
