COVID-19 Vaccination Event

Anyone 12 Years and older

Wednesday, May 26, 2021

12:00pm - 2:00pm

Students 12 years and older, family members, and community members are welcome to get vaccinated against COVID-19 at outdoor sites next to the East Somerville Community School, West Somerville Neighborhood School, or Kennedy School.

Consent form for Students Under 18

Signed parental consent is required for students 12-18 years old. Parents/guardians are not required to be present, but are encouraged to join their students and are also welcome to receive their vaccination at the same time. If your student will attend the event alone, please send them with a signed consent form (see below).
