You can fill out the secure form below to complete your application for any of the scholarship available through the Somerville High School Scholarship Foundation. When you are done your application will be emailed automatically to a representative of the Somerville High School Scholarship Foundation.

If you are more comfortable printing a copy of the form and completing it neatly by hand please use the attached PDF. If you are printing and completing the form by hand please note you must complete a separate application for each scholarship you are applying for. To make it easy, consider filling out the entire form EXCEPT the "Name of scholarship" part. When you have finished everything but that, photocopy as many of the forms you need. (For example, if you are applying for three scholarships make three copies. Then you have three completed applications but you only had to fill in the form once.) Then on each of the copies write the name of the individual scholarship for which you are applying.
